About The Position

This employee will provide case management services to adults with a diagnosis of a severe mental illness that are working with the AOT Program and Mental Health Court. Duties include: (1) Provide intensive case management to identified clients. Provide essential transitional care coordination services, including pre-release contacts, assessments and service planning, and assistance with, housing, vocational, linkage to community supports (e.g. mental health, substance use, and medical providers), advocacy, and case conferences. (2) Have ongoing contact with community agencies necessary to develop additional resources and coordinate services and successfully connect clients to longer term community support services. (3) Collaborate professionally and effectively with stakeholders, such as the Courts, Salvation Army, Probation and Parole, and state level peers. (4) Must have transportation and be willing to assist clients by providing outings and keeping scheduled appointments with various community advocacy agencies. (5) Employee is also responsible for timely record keeping associated with the services provided. (6) Attend monthly supervision as determined by supervisor. (7) Complete and maintain documentation of a minimum of six (6) hours of training or continuing education each year. (8) Other duties as assigned/required.

Requirements

  • Employee is required to have at least a bachelor's degree in psychology, social work, or related field with at least 1 year of experience in working in a human services setting.
  • Must have a valid driver's license, reliable transportation, and be willing to transport clients on a daily basis.
  • Must present proof of automobile insurance and pass a Criminal Record Check.
  • Must present proof of a recent (less than 1-year-old) TB skin test.
  • Within 6 months of employment, this employee must attend the Core and SMI case management trainings.
