About The Position

Hope For Youth is looking for a Full-Time Intensive Case Manager. Are you passionate about keeping children safe, strengthening families, and preventing out-of-home placements? If so, Hope For Youth wants you to join our dynamic Intensive & Supportive Case Management (ISCM) team! Hope For Youth's Intensive & Supportive Case Management (ISCM) program is an out-patient preventative service that provides children and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County.

Requirements

  • Bachelor's degree in Social Work (BSW) or a related field
  • Valid NYS Driver's License in good standing
  • Reliable personal vehicle for regular home visits and transports throughout Suffolk County
  • Strong writing and time management skills
  • Flexibility to work evenings and occasional weekends
  • Ability to respond to calls or emergencies via an agency-provided mobile phone or internet communication tools
  • Ability to read and write reports/logs
  • Prolonged walking, standing, or sitting
  • Bending, stooping, kneeling, squatting, climbing stairs, and reaching overhead
  • Repetitive wrist/finger movements for documentation
  • Regular driving using a reliable personal vehicle

Responsibilities

  • Maintain a manageable caseload of 4 youth conducting twice-weekly home visits
  • Split time between our Amityville main office and the community with a flexible schedule
  • Identify and address safety concerns and advocate for family needs
  • Collaborate with interdisciplinary teams, Suffolk County Department of Social Services, and other community partners
  • Complete all documentation and reports in a timely manner
  • Attend training and staff development opportunities provided by HFY, including but not limited to: Individual Supervision, Group Supervision, staff meetings, and webinars
  • Respond to all calls/emergencies via agency-provided mobile phone and/or via internet technologies
  • Field work and home/community visits
  • Documentation and case-related paperwork
  • Staff meetings, supervision, and interdisciplinary team meetings
  • Training and professional development
  • Other duties as assigned
