About The Position

The Intensive Case Manager will manage a high-risk caseload with a specific focus on working with complex, intersectional cases of domestic and gender-based violence including, but not limited to, family violence, elder abuse, human trafficking, and sexual violence. The Intensive Case Manager will navigate clients through the many complex systems called into action in service plans, such as behavioral health services, medical services, criminal justice, family court, and child protective services. The Intensive Case Manager will be a strong advocate who can provide ongoing support to clients with the police, prosecutors, housing, shelters, schools, public assistance, and other service providers. The Intensive Case Manager will provide on-going and enhanced follow-up with clients to ensure that clients are accessing and successfully connecting with the array of supportive services available at the FJC and through off site partnerships and linkages, creating positive relationships with on and off-site partners.

Requirements

  • Bachelor’s degree in health, human services, education, social work, or mental health with 1 year of qualifying experience OR Associates degree in related field with minimum 3-4 years of qualifying experience; qualifying experience includes work with individuals at high risk of contracting HIV/AIDS, history of mental illness or substance use, harm reduction, domestic violence, immigrants, LGBTQ, etc.
  • At least 3 years of experience working with culturally diverse and vulnerable populations who have experienced trauma.
  • Must be able to demonstrate the ability to engage and serve the domestic violence population targeted by this program and have strong engagement and follow-up experience.
  • Have familiarity with orders of protection and navigation of the criminal and family court systems.
  • Must have basic knowledge regarding HIV, HCV, STI, PEP/PrEP and Harm Reduction methods.
  • Excellent MS Office (Outlook, Word, Excel, PowerPoint, Teams) and internet navigation skills.
  • Ability to work independently and as a team; be team player with an ability to work with diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates others to perform at their best.
  • Must possess the ability to interact with all levels of external and internal partners with a high level of professionalism and tact in representing CHASI.
  • Strong verbal and written communication skills, including capability to listen and follow instructions; able to give, provide direction and follow up on task; high attention to detail and accuracy.
  • Be highly organized and able to multitask.

Nice To Haves

  • Fluency in Spanish highly preferred.

Responsibilities

  • Conduct initial comprehensive assessments, safety planning, and risk assessments with clients.
  • Navigate the client to execute their service plans, including making active referrals, accompany clients to various appointments, provide coaching to clients, deliver weekly or monthly education encounters, plan logistics for plan adherence reminders, conduct regular follow-up, and assist with transportation to get to and from appointments.
  • Provide trauma-informed education about domestic violence and all forms of gender-based violence.
  • Contact clients via phone, text, and email should they fail to attend appointments or stray from documented service planning to re-engage the client.
  • Work collaboratively with internal departments and external partners.
  • Collect and submit data.
  • Document all case management activities in accordance with guidelines and standard procedures.
  • On a weekly basis, review and update the program client list, deliverable tracker, and internal database.
  • Participate in internal and external meetings and trainings as assigned.
  • Align all activities with CHASI’s mission, core values, and culture.
  • Other duties as assigned.
