As our Intensive Case Manager, you will engage the client in implementing an Individual Recovery Plan (IRP), based on their self-identified goals and objectives, service needs, strengths, barriers, support systems, and emotional triggers. Using this plan, you will provide support and training, and link the client to community services to assist with identified needs. Through regular community visits, you will monitor the client’s progress and need for additional supports to decrease hospitalizations, incarcerations, and episodes of homelessness, and increase their housing stability and employment and community engagement. This role works with clients’ doctors, residential providers, and other services involved to provide wrap around support.
