About The Position

At Bethesda House, our goal is to end the cycle of poverty and homelessness - not just manage the symptoms. Our teams work to address each individual on a holistic level so they feel heard, respected, and safe in our care. We offer hands-on help to address chronic homelessness, mental health, substance abuse, disability, and systematic poverty by offering innovative solutions and proactive services to the most vulnerable in Schenectady County. We provide integrated primary care and behavioral health treatment programs and assist with providing access to financial and educational resources where applicable. Be the bridge from crisis to stability. The Intensive Case Manager (ICM) to provide hands-on outreach and crisis support to individuals in emergency shelters, including hotels and motels. In this fast-paced role, you’ll work closely with local partners to quickly move individuals from temporary placements into stable housing while addressing critical health and social needs. Onsite, full-time (40 hr/week), Non-exempt position Pay Rate: $23/hour based on individual skills and experience Schedule: M-F, 8am-4:30pm

Requirements

  • Experience in homelessness, crisis response, or intensive case management
  • Strong communication, organization, and problem-solving skills
  • Ability to thrive in a fast-paced, field-based environment
  • Knowledge of community resources and housing systems

Nice To Haves

  • Experience with HMIS or similar data systems preferred

Responsibilities

  • Provide on-site support in emergency shelters and hotel placements
  • Conduct intakes, assessments, and immediate service planning
  • Deliver intensive, hands-on case management and advocacy
  • Partner with Schenectady County DSS to expedite housing placements
  • Coordinate appointments and attend with clients as needed
  • Support rapid transitions from shelter to permanent housing
  • Work closely with Social Work and Medical teams to address whole-person needs
  • Build strong relationships with community providers and housing resources
  • Maintain accurate documentation and HMIS data (Coordinated Entry, ES, CM)
  • Complete required reports and ensure compliance with all regulations
  • Prepare for audits and maintain organized client files
