About The Position

The Clinical Case Manager II – Intensive Case Management (ICM) is a dynamic, knowledgeable, and experienced professional with a background in social work, psychology, or behavioral health. This role is ideal for someone passionate about serving individuals with complex needs—including serious mental illness, substance use disorders, housing instability, and trauma—who are also involved in the criminal legal system. The Clinical Case Manager II will join an innovative, multidisciplinary team that is piloting a supportive and integrated, community-based service delivery model. This model brings together behavioral health clinicians, peer specialists, housing specialists, and client care coordinators to promote better outcomes for a historically underserved population. The team serves both youth and adult clients (ages 16 and older), providing a unique opportunity to build clinical skills across the lifespan. This is an exciting opportunity for clinicians seeking to deepen their trauma-informed practice, gain experience in justice system navigation, and earn clinical hours toward licensure. Requirements

  • MSW or relevant master's degree required with 2 years of related case management experience
  • LMSW, LMHC, or LCAT licensure required
  • Familiarity with the Alternative to Detention (ATD), Alternative to Incarceration (ATI) or other advocacy program models
  • Working knowledge of treatment programs (substance abuse, mental health, etc.) and other support services (GED, job training, YIP, etc.) for referrals
  • Ability to exercise sound and quick judgment
  • Excellent communication and writing skills
  • Excellent time management skills
  • Computer literate with the ability to quickly adapt to changing technology
  • Must be willing to work evening and weekend hours, if needed
  • The ability to work in-person 5 days a week.

Nice To Haves

Responsibilities

  • Conduct comprehensive intake and needs assessments using evidence-based tools to identify mental health, substance use, housing, and other psychosocial needs.
  • Provide trauma-informed, person-centered clinical case management to a caseload of high-risk, high-needs youth and adults.
  • Collaborate with a multidisciplinary team to develop and implement individualized service plans that promote stability, recovery, and court compliance.
  • Support participants in navigating complex systems, including behavioral health, housing, public benefits, and legal services.
  • Maintain regular contact with participants through in-person and phone check-ins; monitor progress, re-arrests, and compliance with court mandates.
  • Create safety, provide supportive counseling, and encourage client growth and readiness for change.
  • Prepare and present written progress reports to the court; advocate for participants’ needs and strengths in legal settings.
  • Coordinate with defense attorneys, service providers, and community partners to ensure continuity of care.
  • Maintain accurate and timely documentation in the agency’s case management system.
  • Participate in ongoing training, reflective supervision, and team meetings to support professional growth and service quality.
