About The Position

SUMMARY: As part of a forensic services team, provide intensive community based services and case management to patients with co-occurring mental health and substance use disorders who are under the jurisdiction of Psychiatric Security Review Board (PSRB.)

Requirements

Must have a Bachelor's Degree in a related behavioral sciences field, or a combination of at least three year's work, education, training or experience.
Must have 2 years of recent related case management and counseling experience with a similar patient population.
Must understand mental health assessment, treatment and service terminology and to be able to apply the concepts. Must be able to provide psychosocial skills development and implement interventions prescribed on a treatment.
Must be able to work independently and as part of a team.
Must be able to work flexible work hours as dictated by patient needs.
Knowledge of regulation and laws governing mental health and addictions services, ethics, confidentiality and other related areas.
Ability to assess patient needs and establish linkage between patients and resources in a coordinated, effective, and efficient manner.
Ability to establish rapport quickly, maintain appropriate boundaries, and remain nonjudgmental to patients.
Demonstrate knowledge of accepted principles of patient record management.
Ability to conduct continuing care, relapse prevention, and discharge planning with the patient and involved significant others.
Awareness of surroundings, and the ability to recognize situations that create safety concerns, disrupt the treatment environment, breach confidentiality, or otherwise conflict with CODA policy.
Ability to de-escalate patient situations through non-violent crisis prevention intervention.
Ability to utilize both oral and written communications skills to effectively communicate with patients, co-workers, community resources, supervisors, and various other individuals encountered in the course of performing work while maintaining patient confidentiality and establishing professional rapport.
Ability to operate a computer, utilize basic MS Office programs, and effectively navigate and maintain patient records in an electronic medical records system.
A valid Oregon or Washington Driver's License.
Must be able to be credentialed as a QMHA upon hire.
May be required to attain a Basic Life Support (BLS) certification or ability to acquire within 90 days of hire.
Crisis prevention training completion within 90 days of hire.

Responsibilities

Supervise patients using a strength-based and motivational approach; to implement the treatment plan and monitor compliance with Conditions of Release.
Assist Intensive Case Manager II (ICM II) in providing case management, skills training, supervision and monitoring of patients.
Provide coordination of care with community partners and adjunct service providers to link patients to housing, employment, health care and other resources.
Provide home and community monitoring in accordance with PSRB Conditions of Release including medication administration and UA procedures as needed to ensure patients meet Conditions of Release. Use CODA-owned vehicle for daily appointments.
Monitor and track patient progress as part of a multi-disciplinary team to include consultation with CODA's licensed medical provider and CODA's Manager of PSRB Services. Ensure that patients follow their conditions of release.
Assess and manage risk of patient harm to self or others and relapse. Seek appropriate clinical support to manage risk.
De-escalate patient situations through non-violent crisis prevention intervention.
Adhere to regulatory requirements and maintain professional, ethical and behavioral standards of conduct.
Attend all meetings as assigned.
Provide timely and accurate clinical documentation including chart notes in compliance with agency and regulatory standards. Maintain the integrity of the electronic medical records system within individual scope of practice.
Consistent attendance and punctuality are required of this position.
