Intensive Care Nurse Practitioner / NP or Physician Assistant / PA
About The Position
Expanding practice seeks an Inpatient Palliative Care NP to work in Findlay, Ohio! Join a collaborative, mission-driven Palliative Care team, working alongside experienced NPs, a Hospice Medical Director, and a Palliative Care Medical Director in a supportive, interdisciplinary environment. This is a full-time inpatient position, with 75% of the time spent at one hospital in Findlay, OH and 25% at a second hospital in Lima, OH. The role involves a Monday-Friday schedule, 8:00 AM–4:30/5:00 PM, with shared evening and weekend telephone call rotation. Providers will perform inpatient palliative care consultations and follow-up visits, manage symptoms for patients with serious, chronic, and life-limiting illnesses, facilitate goals-of-care discussions and advance care planning with patients and families, assist with discharge planning, including transitions to home-based palliative care or hospice, and provide inpatient rounding for hospice General Inpatient (GIP) patients at the second hospital as needed. Requirements
- Certified Nurse Practitioner (Acute Care NP preferred; palliative care or hospice experience welcomed)
- No experience needed!
- Strong communication, interpersonal, and organizational skills
- Comfortable navigating complex conversations related to serious illness, prognosis, and end-of-life planning
- Ability to work independently while collaborating within an interdisciplinary team
