About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking two (2) Intensive Care Managers to join our team. This role collaborates with CHNCT’s clinical team, provider network, DSS and the community to create a process of assessment, planning, coaching and facilitation to meet the member’s health care needs across the continuum of care. The Intensive Care Manager assists the member by functioning as an advocate for options and services by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy. The role involves engaging with members through telephonic, videoconferencing and face-to-face outreach, and meeting with providers and community organizations to coordinate care. The Intensive Care Manager conducts assessments to identify the member’s needs and develops a specific care plan to address objectives, barriers and behaviorally specific goals identified during the assessment. They must maintain knowledge of covered benefits for CT Medicaid and HUSKY Health programs.

Requirements

Two to four years post-secondary schooling required
Associate's degree, Bachelor's degree preferred
Nursing (required)
Minimum two years direct clinical care experience as an RN is required.
Active Registered Nurse License
State motor vehicle license
Excellent verbal and written communication skills.
Good time management and organizational skills along with the ability to manage continually changing priorities.
Strong computer and keyboard skills.
Ability to exercise independent and sound judgment in decision making.
CT RN License must be current and unrestricted.
Must hold valid and unrestricted motor vehicle operator’s license and have reliable transportation.

Nice To Haves

Behavioral health experience and knowledge of case management are beneficial.
Managed care and case management experience preferred.
CCM or other URAC approved certification preferred.

Responsibilities

Collaborates with CHNCT’s clinical team, provider network, DSS and the community to create a process of assessment, planning, coaching and facilitation to meet the member’s heath care needs across the continuum of care.
Assists the member by functioning as an advocate for options and services by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy.
Develops, monitors and evaluates the effectiveness of the care plan and modifies as necessary.
Provides education and coordination of outpatient care to CHNCT members selected for Case Management.
Identifies and refers quality of care, coordination of benefits and third party liability issues as appropriate.
Orients new staff as requested.
Performs other duties as assigned.
