The Intensive Care Coordinator (ICC) is responsible for coordinating individualized, community-based services for children and adolescents with Serious Emotional Disturbances (SED). Using a wraparound, family-centered approach, the ICC facilitates team-based planning, service coordination, and cross-system collaboration to support youth and families in achieving stability, improved functioning, and meaningful life goals. The ICC ensures that care is well-integrated across providers and tailored to each youth's strengths, needs, and preferences. This position may involve working with youth at high or low intensity levels, depending on the complexity of their needs.

Requirements

  • Coordination of individualized, community-based services for children and adolescents with Serious Emotional Disturbances (SED).
  • Use of a wraparound, family-centered approach.
  • Facilitation of team-based planning, service coordination, and cross-system collaboration.
  • Ensuring integrated care across providers, tailored to youth's strengths, needs, and preferences.
  • Working with youth at high or low intensity levels.
  • Development and implementation of individualized care plans using a high-fidelity wraparound process.
  • Facilitation of Child and Family Team (CFT) meetings.
  • Conducting comprehensive assessments and strengths-based planning.
  • Acting as a primary point of contact and advocate for youth and families.
  • Coordinating and monitoring services and supports across multiple systems and providers.
  • Timely documentation of assessments, service plans, progress notes, and communication.
  • Identifying and addressing gaps in services, advocating for needed resources.
  • Promoting culturally responsive and trauma-informed care.
  • Maintaining regular contact with families and providers.
  • Monitoring progress, promoting engagement, and supporting family voice and choice.
  • Participation in supervision, team meetings, and ongoing training.

Responsibilities

  • Lead the development and implementation of individualized care plans using a high-fidelity wraparound process.
  • Facilitate regular Child and Family Team (CFT) meetings with families, service providers, and natural supports to coordinate care and revise service plans as needed. (For High Intensity Youth only).
  • Conduct comprehensive assessments and strengths-based planning with youth and families.
  • Act as the primary point of contact and advocate for youth and families in navigating mental health, educational, legal, and social service systems.
  • Coordinate and monitor services and supports across multiple systems and providers.
  • Ensure timely documentation of assessments, service plans, progress notes, and communication in compliance with agency and regulatory standards.
  • Identify and address gaps in services, advocate for needed resources, and promote culturally responsive and trauma-informed care.
  • Maintain regular contact with families and providers to monitor progress, promote engagement, and support family voice and choice throughout the care planning process.
  • Participate in supervision, team meetings, and ongoing training.
