Uses independent judgment and discretion to provide direct services including case planning, advocacy, outreach, and home visitation activities for children and families in collaboration with a team. This role involves conducting regular home visits to determine client needs, providing training, guidance, and support to families, and participating in crisis intervention. The coordinator also provides casework and support services, makes referrals for therapy, coordinates with community services, and advocates on behalf of clients. Maintaining accurate client files, representing the organization at meetings, and establishing effective working relationships are key aspects of the position. The role requires extensive travel within LA County and occasional travel beyond.
