Integrative Health Provider (Contracted)

Indiana Wesleyan University•Fort Wayne, IN

About The Position

The Integrative Hub is seeking a credentialed, mission-aligned Integrative Health Provider to join its care team as an independent contractor. The provider serves as the clinical anchor of a coordinated, non-prescribing Direct-to-Consumer clinic, working alongside the Health Coaching team. This is a focused, relationship-driven practice rooted in evidence-based lifestyle intervention, not a primary care or urgent care role. The provider conducts comprehensive lifestyle and health history intakes, orders and interprets standard lab panels, and develops individualized treatment plans across the six pillars of Lifestyle Medicine: nutrition, physical activity, sleep, stress management, avoidance of risky substances, and positive social connection. Through follow-up visits, the provider assesses progress and adjusts plans, offering health education and motivational interviewing. The provider coordinates with patients’ primary care providers and recommends referrals to specialists or other care settings as appropriate. The clinic serves adults 18 and older. At launch, the role excludes prescribing medications; urgent or emergent care; mental health diagnosis or psychiatric treatment; serving as a patient's primary care provider; ordering imaging; in-clinic procedures such as injections or IV therapy; and pediatric care. The provider practices autonomously within the defined non-prescribing scope. Health Coaches receive clinical notes through the EMR (eClinicalWorks), so clear documentation is essential. The provider is engaged as a 1099 independent contractor, maintains active malpractice insurance (reimbursed by IWU as part of the contractor arrangement), and manages scheduling in coordination with clinic availability.
