About The Position

This position is responsible for managing an assigned caseload of clients having long-term mental illness or substance use issues and for assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs.

Requirements

  • Working knowledge of the nature of serious mental health diagnoses or substance use disorders.
  • Working knowledge of treatment planning.
  • Ability to maintain detailed records.
  • Ability to establish and maintain positive professional relationships.
  • Bachelor's degree in relevant human services field or equivalent experience.
  • Must comply with all requirements and provisions of Subpart D, of 43 CFR, Part 2, which implements the Privacy Act (5USB, 552a) (i)(1).
  • Must consent to pre-employment drug testing prior to appointment, and abide by the Drug-Free Workplace Policy while employed by the Kickapoo Tribe in Kansas.
  • Undergo a criminal background check.

Responsibilities

  • Participates in the screening of new service requests.
  • Participates in regular interdisciplinary staff meetings.
  • Completes case management assessment on all clients entering caseload.
  • Develops appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients.
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan.
  • Documents all service contacts on a timely basis.
  • Participates in interagency planning and service coordination activities.
  • Meets regularly with the Project Director/ Integrated Care Manager as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
  • Works collaboratively with and provides clinical consultation within the department.
  • Performs other duties as assigned by the Project Director/Integrated Care Manager which are consistent with the position and in compliance with agency policies and procedures.
  • Must be able to travel for local and out-of-state conferences and trainings for professional development.
