About The Position

Vanna is seeking Integrated Health Coaches with a deep passion for supporting individuals with behavioral health challenges as they build fulfilling, connected lives within their communities. Coaches are responsible for locating and engaging members, building trusting relationships, and providing exceptional coaching, advocacy, and support throughout their recovery journey. This partnership begins with understanding each member’s personal goals and connecting them to the right resources to achieve those goals. Coaches play a critical role in both physical and behavioral health coaching, collaborating closely with clinicians and specialists to ensure members receive coordinated, high-quality care. We believe strongly in the power of human-to-human interaction. Coaches meet members where they are within their communities, at their homes, and through phone, text, and email. Your insight and lived experience will be deeply valued as we continue improving the member experience, informed by the voices of our population and the communities that support them.

Requirements

  • Minimum of 2 years of experience working directly with individuals living with serious mental illness
  • Experience working within Long Island communities
  • Excellent verbal and written communication skills
  • Strong organizational and time management skills with the ability to work independently in a remote environment
  • Comfortable using technology and learning new software quickly
  • Willingness to become proficient in Google Workspace (Gmail, Calendar, Docs), Zoom, Slack, and other required technology platforms
  • High school diploma or equivalent
  • Driver’s license and willingness to use a personal vehicle
  • Ability to commute to Manhattan for day 1 of training (travel cost will be reimbursed in accordance with Vanna’s travel policy)
  • Dedicated home office with a private, HIPAA-compliant workspace and reliable high-speed internet for long-term remote work
  • Ability to travel throughout Nassau County using a personal vehicle to meet with members in the community
  • Ability to spend a significant portion of the workday in the field meeting members, as needed

Responsibilities

  • Conduct initial outreach to eligible members through phone, virtual, and in-person interactions
  • Engage and build trust: Locate and engage members in their communities, establishing strong, collaborative relationships with members and their care teams
  • Achieve enrollment goals: Meet daily enrollment and engagement targets to support program growth and member impact
  • Support recovery and self-determination: Partner with members to develop, track, and achieve personal recovery goals
  • Provide role modeling and encouragement: Serve as a consistent, positive presence throughout each member's recovery journey
  • Facilitate wellness and coping strategies: Encourage healthy behaviors and coping strategies that promote physical and behavioral health
  • Deliver physical health coaching: Provide physical health coaching in accordance with Vanna training and program guidelines
  • Drive continuous improvement: Share feedback with Vanna's technology and operations teams to improve workflows and the member experience
  • Maintain accurate and timely documentation while complying with HIPAA, health plan requirements, and Vanna Health policies
