About The Position

This position is responsible for the follow-up to clinical health reviews with individuals and their teams. Also, responsible for collecting outcome data on follow-up and progress in implementing recommendations, as well as monitoring improvement in the health status of individuals reviewed. The Integrated Health Care System Navigator will develop and provide training (on-line and in-person) and consultation to individuals, families, providers, supports coordinators, and team members on any behavioral health or dual diagnosis topic. This individual is required to work with all Southeast Region Administrative Entities and the Office of Developmental Programs (ODP) on SE Regional or state-sponsored Dual Diagnoses (DDX), meaning mental health and intellectual developmental disabilities (IDD) projects and workgroups.

Requirements

  • Knowledge of community physical and behavioral health, and ODP/OMHSAS system resources
  • Strong written communication skills
  • Bachelor’s degree with 3-5 years’ experience in Human Service, Psychology, Behavioral Health
  • Previous working experience with Intellectual and Developmental Disabilities/Autism and Dual Diagnosis (mental health and I/DD)
  • Training development and presentation experience (on-line and in-person)
  • sitting for a minimum of 2 hours; standing for a minimum of 20 minutes concurrently throughout the day;
  • Ability to travel throughout the Southeastern Region of Pennsylvania;
  • Regularly requires walking, standing, stooping, bending, sitting, reaching, pushing, pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day;
  • Ability to utilize computers and office equipment to complete daily work responsibilities;
  • Viewing computer screen(s) for extended periods of time;
  • Must be able to work in an office environment that may include varying levels of distraction, noise, lighting, and privacy;
  • Must be able to adapt to continuous changes/demands of the job.

Responsibilities

  • Serve as a key clinical partner with the PCHC behavioral health team, contributing expertise to improve care coordination and person outcomes.
  • Follow up on recommendations made by the Behavioral Health Clinical Integrated Health Review (IHCR).
  • Participate in the Integrated Health Clinical Review (IHCR) meetings.
  • Conduct 60 and 120 day Follow up Meetings to ensure implementation of recommendations. Document meetings and follow up strategies.
  • Conduct longitudinal follow up at the six month and one-year marks for individuals that have completed the IHCR process.
  • Work closely with the PCHC Quality Administrator to track outcomes for the individuals seen via outcome tracking tools and report results as requested.
  • Complete the train the trainer online training related to the Dual Diagnosis Curriculum through MyODP, to be a resource on the topic throughout the Southeast Region and train on the curriculum.
  • Participate in Team Meetings with individuals, agencies and families, as needed.
  • Participate in Behavioral Health Intellectual Disability (BHID) Implementation activities.
  • Responsible for creating articles for the PCHC website and Health Care Alert newsletter related to behavioral health, integrated health and health outcomes.
  • Provide training and technical assistance as requested from agencies, families or at regional events.
  • Participate in county Human Rights Committees (HRC’s) monthly.
  • Participate in any ODP Dual Diagnosis Activities and workgroups
  • Other duties as assigned
