About The Position

The Integrated Care Team Lead is responsible for coordinating, implementing, and providing general oversight of the outpatient integrated health service delivery provided by an array of staff members. They will provide assessment, counseling, and support services to an assigned caseload. This role involves cooperation with members of the treatment team and overseeing the health and safety of the treatment environment. The Team Lead will establish good rapport with a wide variety of agencies, organizations, and individuals. They will work closely with treatment team personnel to facilitate admission and pre-discharge planning, care coordination, and team communication. A key responsibility is to lead, coach, and foster professional growth for team members.

Requirements

  • A Masters degree in Counseling, Social Work, Psychology, or related field OR Active LIMHP and LADC licenses in the State of Nebraska
  • At least 21 years of age
  • Experience working with persons with co-occurring and complex diagnoses
  • 5 or more years' experience in a related field
