The Care Team Lead is responsible for the day-to-day leadership, oversight, and development of an assigned team of Patient Coordinators. Reporting to the Care Team Director, this role ensures the consistent execution of care coordination processes, promotes operational excellence, and supports the delivery of exceptional patient and caregiver experiences. The Care Team Lead provides daily coaching, guidance, and accountability to Patient Coordinators while monitoring productivity, quality, documentation, and service standards. This position serves as the first point of escalation for routine operational concerns and partners closely with the Care Team Director to achieve departmental goals and drive continuous improvement.
