About The Position

ESSENTIAL JOB FUNCTIONS Make appointments and/or referrals for PCP or Medical Specialty. Maintain the Integrated Care Coordinators schedule including appointment reminders, scheduling appointments and rescheduling following a cancellation or no show. Prepares and mails caring contacts to clients determined to be at a risk of suicide as directed by Integrated Care Coordinator. Assists clients in navigating the healthcare environment to ensure appropriate access to care. Assists with coordination/access to services that address social determinants of healthcare outcomes: Assist with options for transportation to healthcare appointments Assist client in obtaining housing resources Assists with access to other community resources e.g., public assistance and financial benefits Develops knowledge base of other healthcare providers in the service area and collaborate, as needed Documents tracking for appointments, medication, and other healthcare needs for assigned clients, as needed.

Requirements

  • High School diploma or GED Preferred, but not required
  • training as a Medical Assistant
  • One year of experience working in a hospital, physician office, or behavioral care clinic
  • Experience in community setting or integrated care environment.
  • Reliable transportation for travel between clinic locations if needed.
  • COVID and Influenza vaccinations for all employees.
