About The Position

The Integrated Care Program Manager provides leadership and oversight for the development and long-term sustainability of integrated early childhood, behavioral health, and pediatric systems. This role focuses on strengthening cross-sector partnerships, building sustainable service delivery models, supporting workforce development initiatives, and improving coordinated access to care for young children and families. The position plays a key key role in advancing systems change efforts, strengthening community partnerships, and embedding sustainable practices across organizational operations.

Requirements

  • Master’s degree required in Public Health, Social Work, Public Administration, or related field.
  • 3 years of experience in public health, behavioral health, systems coordination, or program management
  • Minimum of one year assessment, advocacy and case management work in a Human Service field required.
  • Minimum of 3-5 years’ experience working with low-income individuals or families required.
  • Desire to work with low income, culturally diverse, and at-risk families and individuals.
  • Strong leadership skills, ability to motivate staff.
  • Excellent customer service, organizational, oral, and written communication skills.
  • Ability to manage multiple priorities and work in a fast-paced environment.
  • Proficient computer skills required in basic computer software and data collection systems.
  • Excellent crisis management skills.
  • Ability to maintain accurate records and confidentiality of information.
  • Able to pass CORI and EEC Background which includes fingerprinting.
  • Ability to show MMR immunity.

Responsibilities

  • Lead ongoing coordination and continuous improvement of integrated pediatric, behavioral health, and early childhood support systems.
  • Support development of sustainable service models, operational structures, and community-based care pathways.
  • Guide program initiatives, which may include caregiver and workforce support.
  • Support family engagement strategies that strengthen caregiver participation, peer support, and coordinated care.
  • Build and maintain collaborative partnerships across healthcare, behavioral health, early childhood, academic, and community sectors.
  • Facilitate consortium activities and cross-sector collaboration to strengthen systems alignment and service integration.
  • Develop coordinated workflows, referral pathways, and Medicaid-aligned approaches to service delivery.
  • Support development of scalable and replicable models that improve access to integrated care and family support.
  • Oversee data collection, evaluation, and continuous quality improvement efforts to inform systems improvement and long-term planning.
  • Support development of sustainable funding strategies, contracts, and billable service models.
  • Monitor program outcomes and utilization trends to support organizational and community impact goals.
  • Prepare reports and presentations for funders, policymakers, and community stakeholders.
