About The Position

The Integrated Care Clinician will focus on integrating care for patients between medical and behavioral health services, working closely with both departments. This role involves monitoring pre-visit medical screens, providing brief evaluations and interventions, including suicide risk assessments, and referring patients to Behavioral Health Services (BHS) for follow-up treatment and case management. The position is under the direction and supervision of the Director of Behavioral Health and requires providing both direct services and specialized supportive services to children and adults. The role requires the ability to work a variable schedule to meet client and program needs and must be sensitive to the Indian culture and its needs, as the position is grant-funded and involves working with the Indian Community.

Requirements

  • Master’s degree from an accredited school that fulfills the academic requirement for Master’s degree in Social Work, Marriage and Family Therapy, Counseling, Psychology, from an accredited college or university.
  • Current, active, and unrestricted California license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC), OR be registered with the California Board of Behavioral Sciences (BBS) as an Associate Clinical Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), or Associate Professional Clinical Counselor (APCC) and working toward full licensure.
  • Eligibility to accumulate clinical hours toward full licensure and be qualified to be licensed within two years of employment under appropriate clinical supervision.
  • A valid California Driver’s License with a safe driving record, DMV record required.
  • Must be insurable by the program’s insurance carrier.
  • Behavioral health experience with integrated care and/or primary care is preferred.
  • Experience providing behavioral health services in a community health, primary care, or integrated care setting is preferred.
  • If a recovering substance abuser, have maintained a minimum of four years of continued abstinence from mood-altering chemicals.
  • If a recovering substance abuse family member (i.e., spouse, child), a minimum of four years of continued involvement in a recognized support system.
  • Must have current CPR certification that complies with the guidelines set by the American Heart Association or the American Red Cross. Certification may also be obtained prior to orientation.
  • Ability to function effectively as a clinician.
  • Ability to provide personal transportation.
  • Ability to work independently when required yet function as a team member with other agency staff.
  • Ability to work on a flexible schedule, which may include early or late shifts, or work on weekends as determined by the Behavior Health Services Director.
  • Ability to share professional knowledge with clinical staff and applying this knowledge to a wide range of complex and difficult cases involving children, adolescents and adults in individual or group counseling services.
  • Must be able to work with the Indian Community, and be sensitive to the Indian culture and its needs.

Responsibilities

  • Develops and provides BHS treatment process for assigned clients, assuming responsibility for a specific caseload.
  • Provides psychotherapy, counseling, and other behavioral health services (individual, group, marital, and family therapy) at assigned clinics.
  • Interviews, evaluates, and makes treatment recommendations for assigned clients based on their medical, psychological, social, and chemical dependency/abuse needs.
  • Provides crisis mental health and chemical dependency/substance abuse intervention, responding to crisis situations on an "as occurring" basis.
  • Participates in the BHS Department "on-call" schedule as assigned.
  • Completes and maintains all client documents, including charting, in a timely and effective manner, ensuring all required documentation and consents are completed prior to treatment.
  • Records client response to treatment and progress in the SOAP format, completing the "Problem List," "Master Treatment Plan," and "Mental Health History Form" in a timely manner.
  • Involves client/family in the treatment process, providing appropriate treatment orientation, information on treatment concepts, objectives, and goals.
  • Develops the initial treatment plan in conjunction with the client, parent, or guardian, and involves them in all periodic updates.
  • Provides clients and families with a clear understanding of confidentiality guidelines and orients them to the program schedule and grievance processes.
  • Encourages clients and their families in following medical treatment recommendations.
  • Responsible for referral of clients to appropriate providers or outside service providers, presenting client needs to BHS professional clinical staffing for review and referral.
  • Assists clients or families with necessary financial assistance referrals and eligibility determination.
  • Discusses with clients and makes appropriate professional and placement referrals in harmony with current BHS guidelines.
  • Refers clients and/or clients' families to Chemical Dependency Counselor assessment and treatment when involved with Chemical Dependency issues.
  • Assists clients and/or agency and community health care professionals with referral and placement of clients in extended care facilities.
  • Establishes and provides ongoing linkage between the agency and clients with community social resource agencies.
  • Reports all suspected cases of child abuse in a timely manner and actively follows up with the client and/or family, presenting all reported cases to the Child Protection Team.
  • Is a proactive participant in Child Protection Team meetings, presenting and providing input regarding child welfare issues.
  • Proactively involved in Behavioral Health Services prevention programs in the RSBCIHI catchment area communities.
  • Coordinates with appropriate agencies for ongoing services of abused Indian children and their families.
  • Coordinates with appropriate agencies the placement of Indian children needing foster or adoptive care.
  • Provides crisis and individual mental health and chemical dependency/substance abuse counseling to clients identified as victims of crime.
  • Prioritizes schedule to accommodate referrals from Tribal Victim Services staff.
  • Provides referrals for appropriate resources (medical, substance use, resources specific for victimization).
  • Meets regularly with Tribal Victim Services staff to discuss and improve upon services provided.
  • Attends trainings specific for various types of victimization (domestic violence, sexual assault, Missing & Murdered Indigenous People, etc.).
  • May provide presentations regarding mental health/substance abuse and victimization.
  • Works alongside behavioral health staff to ensure mental health, substance abuse, and victimization referrals are being met.
  • Completes objectives and reporting required by grant funding.
  • Provides ongoing treatment for the emotional needs of chemically dependent clients and/or their families.
  • Works in conjunction with Chemical Dependency Counselors in providing treatment of the emotional needs of chemically dependent clients.
  • Provides treatment to family members of chemically dependent clients to break the co-dependency cycle and develop a positive family environment.
  • Maintains a positive regard for and a working knowledge of AA/NA/AL-Anon and other 12-step support groups and programs.
  • Actively seeks supervision, using the supervisory process appropriately based on professional needs.
  • Accepts supervisory guidance constructively, using the direction provided to enhance professional skills or employee performance.
  • Completes all required reports in a timely and acceptable manner.
  • Demonstrates an ability to work effectively with minimal monitoring – a "self-starter."
  • Reinforces the philosophy of RSBCIHI in both professional and clinical areas, including conformance to existing policies regarding personal interests, time, and dress codes.
  • Proactively develops and promotes a working "team relationship" in the Behavioral Health Services Department and with other agency staff.
  • Provides in-service training for other Behavioral Health Services staff or RSBCIHI departments as requested.
  • Is proactive in promoting community knowledge of emotional and mental health problems.
  • Conducts workshops and classes individually and in conjunction with other professional staff members relevant to the Indian communities.
  • Assists with site visits by specific individuals or groups for orientation.
  • Willingly accepts and performs as related administrative tasks as additionally assigned.
  • Accepts other responsibilities as assigned.
