About The Position

The Intake Specialist will be responsible for making initial contact with eligible patients, coordinating community health worker calendars, scheduling appointments, and reviewing hospital census and other data sources to identify eligible patients. This role also involves promoting referrals from community partner organizations, updating health education materials, assisting patients in securing needed medical supplies, and supporting other team members. The Intake Specialist will establish trusting relationships with patients, their families, and community partner organizations, and work cooperatively with other clinical personnel. The position requires the ability to remain calm under stressful situations and perform varied activities, including frequent sitting, standing, and driving throughout the shift.

  • Ability to work independently without constant supervision
  • Ability to effectively communicate with clients, medical personnel, community partners and others they may come into contact with, in a pleasant and constructive manner
  • Ability to prioritize workload in a timely manner
  • Ability to take direction from lead, manager or director regarding work activities and procedures
  • Ability to handle a variable and continuously evolving workload
  • Able to work as assigned every week
  • Have active automobile insurance, driver’s license and ability to drive safely throughout the geographic area defined by Sky Lakes Medical Center, in all types of weather.
  • Must be able to maneuver through unpredictable and unalterable physical spaces
  • Must be able to climb minimum of two flights of stairs.
  • Must be able to work with periods of indirect supervision in patient’s place of residence.

Nice To Haves

  • 1-3 years health/social service experience
  • Experience working in multi-cultural settings
  • Bilingual

Responsibilities

  • Makes initial contact with eligible patients.
  • Accompanies community health workers on home visits.
