The Intake Specialist (IS) conducts in-home assessments and provides information to individuals and their families who are interested in enrolling in the Community LIFE program. The IS explains Community LIFE’s benefits and services, answers questions or concerns, and completes an assessment of medical, psycho-social and environmental conditions to further identify the needs and goals of potential participants and their family caregivers. The IS assists potential enrollees with gathering information needed for clinical and financial eligibility, works closely with Community LIFE’s Medicaid Eligibility Coordinator, and monitors the intake process using a customer relations management system. The IS introduces enrollees to the Interdisciplinary Care Team, providing background information, and coordinating an in-person orientation which occurs prior to enrollment. The IS must have excellent interviewing, record-keeping, writing, and verbal presentation skills. Additionally, he or she must be able to represent the organization professionally to referral sources and the potential enrollee’s family members and caregivers.
