About The Position

The Intake Specialist will assess clients for appropriate placement into SAHLC treatment programs by completing Biopsychosocial, ANSA, SOGA, and health risk screening forms. They will develop an Initial Treatment Plan for clients, ensure all releases are filled out appropriately and have the proper signatures, and transfer clients to the primary clinician and case managers as needed. The role also involves coordinating with external referral sources to accommodate appropriate service needs of clients while ensuring that the Mission of The Salvation Army is effectively carried out.

Requirements

  • Master’s Degree or Licensing Track (License Mental Health Counselor LMHC or License clinical Addiction Counselor LCAC), LCSW (Licensed Clinical Social Worker).
  • Position requires a background check to be completed, and all background check results will be reviewed.
  • Two years of office experience.
  • Must have a valid driver’s license and maintain The Salvation Army Driver’s qualification standard.
  • Must complete Safe From Harm training within the first 90 days of employment.
  • Certified by ICAADA or any other additions certifying body expected.
  • National Certified Peer Recovery Support Specialist or Recovery Coach Certification required or must be obtained within the first 90 days of hire.
  • Able to speak, write and understand English in a manner sufficient for effective communication with leadership, field personnel, and clientele.
  • Computer proficiency with Microsoft products and ability to learn electronic reporting systems.
  • If in recovery, must demonstrate at least two years of sobriety with evidence of emotional and social stability.

Responsibilities

  • Conducts intake/admission including collecting basic demographic and background information, completing SOGA and TCUDS, comprehensive needs, abilities, strengths and preferences, drug use history, and orientation to SAHLC, services and expectations.
  • Conduct assessments for Withdrawal Management clients within 48 hours of admission if the client is medically stable.
  • Enrolls and re-enrolls eligible clients in the DMHA Supported Client or other funding programs by completing the appropriate enrollment forms.
  • Develop and Initial Treatment Plan to initiate treatment for the client.
  • Completes the Biopsychosocial with the client.
  • Completes an action sheet regarding placement of the level of care.
  • Completes a Clinical summary pertaining to clients’ history and presenting problem.
  • Utilizes DSM V to accurately diagnose the client based on the criteria given in the interview.
  • Staff all cases in a weekly multi-disciplinary team meeting to assess the appropriate treatment level of care and diagnosis.
  • Maintains client confidentiality according to Federal regulations and The Salvation Army standards.
  • Attends staff meetings, departmental meetings, seminars/conferences, and in-service training programs as scheduled.
  • Maintains all certifications by meeting certification requirements.
  • Enter all the required client information into the CSM computer system.
  • Other tasks as assigned by leadership.
