Intake Services Family Navigator
About The Position
The Intake Services Family Navigator will complete a comprehensive assessment and provide linkage to community organizations that offer services such as financial assistance, shelter or rapid rehousing programs, educational support, job readiness training, mental health therapy, substance abuse services, wellness services, and medical treatment. Caregivers will complete an initial screening, and if eligible, will be entered into the Navigation program. Eligibility requires a financial need, but assistance will be provided for any other existing needs. Families must commit to meeting with a Navigator to determine needs and create a plan for accessing resources. The Navigator will support families throughout the entire process by fostering a supportive and non-judgmental atmosphere. Cases will be closed once families feel sustained and have obtained their resources, but they can return at any time.
