About The Position

This Full Time position involves working 4 days a week (80 hours per pay period) with rotating weekends on 10-hour shifts. The role is responsible for patient referrals and intake activities, serving as a liaison with area discharge planners, referrers, payers, and patients/families. The Intake RN will facilitate hospice referrals, conduct initial nursing assessments, and arrange for services to begin. This role also involves consulting with the Hospice Physician and/or clinical manager on anticipated needs for new admissions, providing information on hospice care, assessing patient/family interest, determining payer sources, coordinating benefits, and ensuring continuity of care. The position requires ongoing assessment of patient medication needs and effectiveness, collaboration with co-workers, and adherence to proper infection control procedures. Additionally, the Intake RN will complete admission documentation and may be assigned projects related to community and public relations.

Requirements

  • Current Michigan License as Registered Nurse.
  • Possesses and maintains a current CPR certification.
  • Current driver’s license.
  • Knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job.
  • Knowledge of Hospice concept and philosophy.

Nice To Haves

  • BSN preferred with experience in community based care.
  • Certification in hospital & palliative care preferred.

Responsibilities

  • Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment, according to the agencies Notice of Privacy Practices and HIPAA regulations.
  • Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed.
  • Assesses and documents appropriateness for hospice at initial assessment visit.
  • Conducts initial nursing assessment visit and arranges for services to begin.
  • Consults with Hospice Physician and/or clinical manager on anticipate needs for new admissions.
  • Provides information on hospice care and programs to referral sources and patients and families.
  • Assesses patient/family interest in hospice care and services as needed.
  • Determines payer source and coordinates the determination of hospice benefits for the referred patient as needed.
  • Coordinates with discharge planners to assure continuity of care and patient admission to the hospice program as needed.
  • Assesses medication for patient for need and effectiveness on an ongoing basis using comprehensive physical assessment.
  • Works as a team member and collaborates with appropriate co-workers.
  • Demonstrates proper hand washing, asepsis, bag technique and universal precautions.
  • Completes written admission documentation with patients and families as needed.
  • Carries out projects and tasks related to community and public relations as assigned.
