About The Position
FlexStaff is seeking a Registered Nurse (RN) with experience in home care (CHHA) to work full-time as a Nurse Instructor for our client, a certified home care agency, located in Brooklyn, NY. Schedule: Monday - Friday 8:30 am - 5:00 pm. Permanent Position. In this role you will be taking referrals and processing appropriate home care services; as well as maintaining open lines of communication with referral sources and within the company departments.
Requirements
- Active NYS RN License
- Minimum of 2 years of nursing or intake experience in a licensed home care agency (CHHA)
- Knowledge of Allscripts a MUST
- EMR knowledge: HomeCare HomeBase (HCHB) or Careport
Responsibilities
- Screen referrals for clinical appropriateness and payer eligibility
- Triage new referrals and identify urgent clinical needs
- Review medical records, hospital summaries, and current medications
- Verify and obtain physician orders and prior authorizations as needed
- Complete timely intake documentation in the EMR
- Coordinate admission scheduling with patient/family, clinicians, and physicians
- Communicate referral details to the interdisciplinary team
- Educate patient/family on home health services, visit expectations, and patient rights
- Identify and escalate safety risks, infection control issues, or barriers to care
- Arrange or schedule initial assessments (telephonic or in-person)
- Maintain compliance with Medicare, state, and agency admission requirements
- Track referral outcomes, denials, and report trends to management
- Provide telephonic triage or on-call handoff for intake issues (if applicable)
- Train/support intake staff on screening and documentation standards
