About The Position

This is a full-time, remote position for a Licensed Practical Nurse (LPN) to join the Intake Department, working 40 hours per week. The role involves assisting with both Homecare and Hospice referrals. The ideal candidate is self-motivated, tech-savvy, an excellent communicator, a team player, flexible, able to prioritize, and organized. The position requires the candidate to reside in New Hampshire and may require occasional travel to the New London office.

Requirements

Active NH (LPN) Licensed Practical Nurse License
Must reside in New Hampshire
2 years of Health Care experience
Knowledge of medical terminology, familiarity with a medical office setting, and/or triage
Knowledge of Home Health/Hospice
Must be comfortable working in a remote environment
Proof of Flu vaccination
Reliable transportation
Valid driver's license
Automobile insurance
Physical exam within the last year
Two-step TB test

Responsibilities

Accurately enter patient referral information into the electronic medical record system
Accurately enter all incoming patient care orders and notify appropriate team members as necessary
Navigate multiple electronic referral software portals
Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships
Triage all incoming phone calls
Communicate effectively with patients and patient care givers
