This position collaborates with nursing leadership and other support departments to manage patient access, throughput, and capacity. The Intake Counselor/Social Worker proactively manages requests for admission, facilitates the throughput of patients throughout the hospital, and ensures timely turnaround of beds. They accurately collect the data required for the intake of patients from a variety of locations including emergency rooms, physician offices, and referring facilities. This role coordinates all physician-to-physician acceptance calls, participates in and documents all acceptance calls, and works with nursing units and leaders to identify the best available bed placement given patient needs, staffing resources, level of care requirements, and throughput goals. The position determines proper patient admission and verifies alignment between the admission order and the determined admission status, assists with determining patient benefits/eligibility, and reviews and plans for all scheduled admissions daily. They manage and accept all referral calls, communicate incoming referrals, and proactively manage hospital bed capacity. The Intake Counselor/Social Worker communicates bed placement/capacity issues to the Director of Intake and Admissions in a timely manner, actively participates in organizational improvement processes impacting patient placement and throughput, and demonstrates positive and timely communication with internal and external customers. Other functions may be assigned by the Director of Intake and Admissions.
