Become a part of our caring community. This is a part-time position, scheduled for up to 29 hours a week, with availability Monday-Friday between 9 am-6 pm CST. Shifts will be scheduled primarily in the afternoons and may vary. Occasional weekend coverage and 2-3 holidays per year are required, with additional holiday coverage as needed. As an Intake Coordinator, you will report directly to the Intake Supervisor. Your responsibilities will include receiving and responding to incoming calls from referral sources or potential patients, exchanging information to identify patient needs, and determining our ability to meet them. You will record call outcomes, make follow-up calls, and identify alternative community service resources when solutions are not available. A key part of this role is performing initial telephone screening to determine patients' appropriateness for home care services with referral sources, advising branch staff of patient acceptance, and providing all relevant information for a smooth transition to home care. You will also contact referral sources to advise of referral status and provide information to the clinical team. Identifying potential payer sources, verifying benefits, negotiating rates, and obtaining initial authorizations and frequency of visits from case managers are crucial. You will establish primary payers, document conversations with payer sources, access national or regional account information, and communicate this to relevant staff. Collaboration with the Intake team, branch staff, and patients to identify solutions for payor source problems is expected, as is monitoring and tracking referral sources' satisfaction levels and communicating customer service issues.
