Wise Path Recovery Centers is seeking an organized, compassionate, and detail-oriented Intake Care Coordinator to serve as the first point of clinical coordination for individuals entering treatment. This role is essential in creating a welcoming, trauma-informed admission experience while ensuring a seamless transition into care. About the Role The Intake Care Coordinator plays a vital role in ensuring patients experience a smooth, compliant, and patient-centered admission process. Working collaboratively with Admissions, Nursing, Medical, Clinical, Recovery Support, and Utilization Review teams, this position coordinates intake activities, gathers required documentation, facilitates communication between departments, and helps remove barriers to treatment. This position is focused primarily on the admission and intake process while supporting continuity of care throughout the patient's treatment stay as needed.

Requirements

  • High school diploma or GED required.
  • Experience in behavioral health, substance use disorder treatment, healthcare coordination, admissions, case management, or a related healthcare setting.
  • Strong organizational skills with the ability to prioritize multiple responsibilities.
  • Excellent verbal and written communication skills.
  • Proficiency with electronic medical record (EMR) systems and Microsoft Office applications.

Nice To Haves

  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Healthcare Administration, or a related field.
  • Previous experience in residential or inpatient substance use disorder treatment.
  • Experience with patient intake, admissions, or care coordination.
  • Knowledge of trauma-informed care principles and recovery-oriented systems of care.

Responsibilities

  • Coordinate patient admissions from referral through admission.
  • Ensure required assessments, consents, screenings, and documentation are completed accurately and timely.
  • Collaborate with Admissions, Nursing, Medical, and Clinical staff to facilitate efficient patient admissions.
  • Serve as a welcoming, trauma-informed point of contact for patients and families during the admission process.
  • Verify completion of required intake documentation in accordance with organizational policies and regulatory requirements.
  • Assist with scheduling assessments and coordinating internal handoffs between departments.
  • Monitor assigned patients throughout the admission process to ensure timely progression into treatment.
  • Communicate effectively with interdisciplinary team members regarding patient needs and barriers to care.
  • Participate in treatment team meetings and contribute to continuity of care planning.
  • Support Utilization Review by assisting with documentation and coordination related to insurance authorizations and continued stay requirements.
  • Complete accurate, timely documentation within the electronic medical record (EMR).
  • Maintain HIPAA compliance and protect patient confidentiality.
  • Follow all organizational policies, state regulations, federal requirements, and accreditation standards.
  • Promote a trauma-informed, person-centered, recovery-oriented environment.
