About The Position

The Home Health & Hospice Intake and Office Assistant is responsible for supporting patient intake, referral processing, insurance verification, documentation management, billing support, and administrative operations within the Home Health & Hospice department. This position serves as a primary point of contact for patients, families, referral sources, and community partners while ensuring accurate records, timely documentation, and efficient coordination of services.

Requirements

  • High School Diploma or equivalent required.
  • Medical office, healthcare intake, referral coordination, administrative support, or customer service experience preferred.
  • Experience working with electronic medical records and office software preferred.
  • Experience with insurance verification, authorizations, referrals, or healthcare billing support preferred.
  • Customer service and communications skill

Responsibilities

  • Serve as the primary point of contact for patients, families, visitors, referral sources, and community partners, providing professional and courteous customer service.
  • Answer, screen, and route incoming communications; respond to inquiries and direct individuals to appropriate staff and resources.
  • Receive, process and assist in the coordination of Home Health and Hospice referrals.
  • Obtain and verify insurance coverage, authorizations, and other payer requirements to support timely patient admission and service delivery.
  • Maintain accurate and congruent patient records within the electronic medical record systems.
  • Scan, upload, file, retrieve, and track patient records and clinical documentation in accordance with organizational policies and regulatory standards.
  • Monitor documentation workflows and follow up on missing visits, physician orders, signatures, certifications, and other required records.
  • Support clinicians and leadership by ensuring accurate and timely management of patient documentation throughout the episode of care.
  • Assist with billing, charge entry, supply tracking, account reconciliation, and other revenue cycle functions.
  • Prepare, maintain, and update departmental reports, logs, spreadsheets, census information, and other operational records.
  • All other duties as assigned.
