Requirements

Demonstrate the ability to work effectively with children, families, caregivers, community providers, and child welfare stakeholders while maintaining professionalism, confidentiality, and appropriate boundaries.
Ability to work independently, exercise sound judgment, respond effectively to crisis situations, and perform each essential duty satisfactorily.
Bachelor's degree in social work or related human services field (related fields will be considered).
Knowledge of child welfare and family preservation services.
Knowledge of family systems and child development.
Knowledge of case management and service planning.
Knowledge of community resources and support services.
Knowledge of court processes and child welfare regulations.
Knowledge of crisis intervention and risk assessment.
0-6 months case management experience.
Child Welfare Employee License.
Strong written and verbal communication skills, active listening, organization, documentation accuracy, critical thinking, and attention to detail.
Ability to maintain case records and comply with agency, DCFS, and court documentation requirements.
Ability to establish and maintain positive working relationships with children, parents, caregivers, service providers, attorneys, educators, medical professionals, and community partners.
Demonstrates professionalism, empathy, collaboration, and effective interpersonal communication skills.
Ability to assess family functioning, identify safety concerns, recognize barriers to family stability, exercise sound judgment, and make timely decisions that support child safety and family preservation.
Ability to prioritize multiple responsibilities and respond effectively to changing situations.
Proficient Computer Skills In: Outlook, Word, database management software, electronic case management systems, and virtual communication platforms.
Ability to coordinate services, manage a caseload, connect families to community resources, monitor service delivery, and ensure compliance with required timelines and program expectations.
Ability to complete assessments, develop service plans, maintain case records, prepare court-related documentation, and utilize case management systems effectively.
Must display a willingness to adopt trauma-informed interventions, principles, and practices and demonstrate a commitment to ongoing professional development in trauma-informed care.
Recognizes the impact of trauma on children, families, staff, and communities and incorporates trauma-informed principles into service planning, case management, and family engagement activities.
Access to reliable transportation: Required.

Valid driver’s license: Required.

Responsibilities

Meets with families in their homes at various hours of the day as well as on call duties as scheduled in potentially high-risk neighborhoods.
Serves as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
Conducts visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency and well-being.
Develops & implements a plan for delivery of services & supports in collaboration with the family that meets the needs of the children and the family.
Performs case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.
Attends court hearings, prepares and files all court documents within required timeframes.
Willingness to travel as position requires frequent travel; follows agency and state guidelines regarding driving, rules of the road, and safety.
