About The Position

Advocate Sherman Hospital is an acute community hospital in Elgin that has provided high-quality care to Chicago's northwest suburbs since 1888. The hospital features a Level II Trauma Center; it sees more than 60,000 emergency room visits, delivers over 1,700 babies and performs more than 10,000 surgeries annually. Supported by four immediate care centers and 12 Advocate Medical Group sites, Advocate Sherman provides more than 325,000 outpatient visits per year. This role involves providing comprehensive care coordination, case management, utilization management, and discharge planning services. The position requires collaboration with patients, families, physicians, and multidisciplinary teams to ensure safe, timely, and patient-centered care. The incumbent will also serve as a resource and educator on care coordination, discharge planning, utilization management, and regulatory standards, while fostering partnerships with community organizations to support optimal patient outcomes. Age-specific knowledge and clinical judgment are required to meet the diverse needs of patients throughout the lifespan.

Requirements

  • Social Worker: License/Registration/Certification: IL-LSW Eligible, LCSW preferred issued by the State of Illinois
  • Care Manager: Registered Nurse License issued by the state in which the Team Member practices.
  • Social Worker: Master’s degree in social work from a Council on Social Work Education (CSWE) accredited school.
  • 1 year of social work experience in healthcare
  • Care Manager: Bachelor’s degree in nursing.
  • 2 years of clinical nursing experience.

Nice To Haves

  • Inpatient Care Management experience is a plus

Responsibilities

  • Provides professional social work and care coordination services through psychosocial assessment, discharge planning, patient advocacy, and resource coordination.
  • Collaborate with patients, families, healthcare teams, and community agencies to develop and implement safe, timely care plans and transitions across the continuum of care.
  • Facilitates access to post-acute services, community resources, mental health support, advance care planning, and assistance with complex social issues.
  • Promotes continuity of care, appropriate level of care, reduced readmissions, and optimal patient outcomes through effective communication, documentation, and care management.
  • Maintains expertise in healthcare regulations, community resources, and professional social work standards while supporting education, compliance, and ethical patient-centered practice.
  • Provides comprehensive care coordination, case management, utilization management, and discharge planning services by conducting patient assessments, developing care plans, coordinating transitions of care, and facilitating access to appropriate resources and services.
  • Collaborates with patients, families, physicians, and multidisciplinary teams to ensure safe, timely, and patient-centered care while addressing barriers to discharge and promoting continuity across the care continuum.
  • Maintains compliance with regulatory and reimbursement requirements through accurate documentation, utilization review activities, and delivery of required notices.
  • Serve as a resource and educator on care coordination, discharge planning, utilization management, and regulatory standards, while fostering partnerships with community organizations to support optimal patient outcomes.
  • Demonstrates age-specific knowledge and clinical judgment to meet the diverse needs of patients throughout the lifespan.
