The Inpatient RN Care Manager Lead is responsible for conducting complete assessments, establishing appropriate plans, and initiating timely interventions. This role involves effective collaboration and negotiation with patients, families, and the care team to achieve patient and organizational goals related to care needs, choices, and satisfaction during discharge planning and care transitions. The Lead will work as a proactive member of the multi-disciplinary care team to promote best practices. Responsibilities include conducting concurrent and retrospective clinical reviews, providing clinical updates to third-party payers, referring cases to a physician advisor as needed, and collecting data on outcomes and resource utilization. The Lead will also serve as an expert user of care management IT platforms, assist the Manager with Human Resources responsibilities for staff (including interviewing, selection, staff development, performance evaluations, and disciplinary actions), and provide general work direction and guidance to case managers. Additionally, the Lead will act as an educator and resource regarding utilization issues and departmental policies, collaborate with management on program development, assist with internal management of Care Management including caseload oversight, and serve as a liaison with physicians on utilization management and care coordination. The role also involves monitoring assigned areas for efficient use of supplies and equipment and determining appropriate clinical equipment needs. The position requires the ability to demonstrate knowledge and skills necessary to provide age-appropriate care and understand the principles of growth and development over the life span.
