About The Position

MedCap Health is seeking a full-time Inpatient rehab APP with acute care experience to join their team. As a physician-led organization, MedCap Health is dedicated to transforming the clinical landscape of inpatient rehab care. They offer a holistic approach to medical services, with specialties such as pulmonology, nephrology, psychiatry, internal medicine, and more. Collaborating with over 200 skilled nursing facilities across 11 states and several inpatient rehab facilities, they strive to close gaps in care and ensure that individuals receive the high-quality healthcare they deserve. The schedule is designed to promote a healthy work-life balance, ensuring ample time for oneself. The APP will work the day shift, Monday to Friday, with no nights. In this role, the APP's expertise will have a tangible impact on patient outcomes every day. They will be responsible for providing high-quality, effective management for patients with rehabilitation needs. Working closely with the attending physician, the APP will manage acute and chronic conditions, ensuring seamless and comprehensive care. The APP is an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care.

Requirements

  • A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
  • 1+ years of experience in inpatient rehab.
  • Acute Care Experience.
  • Must have the ability to pass a background check.

Responsibilities

  • Providing high-quality, effective management for patients with rehabilitation needs.
  • Managing acute and chronic conditions in collaboration with the attending physician.
  • Communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care.
