About The Position

Senior Doc is expanding its Inpatient Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for patients with serious or chronic illness. This role offers the ideal balance of autonomy and collaboration, allowing you to make a genuine impact while maintaining work–life balance across multiple care settings. Senior Doc brings high-quality medical care directly to patients, restoring dignity, comfort, and trust in healthcare across inpatient, community, and home-based environments. We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions. This position supports inpatient palliative care, community-based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.

Requirements

  • Active California Nurse Practitioner or Physician Assistant license
  • Strong interdisciplinary team leadership skills
  • Excellent time management and organizational abilities
  • Comfortable with home visits and community driving
  • Strong communication skills with patients, families, and care teams
  • Ability to work independently while coordinating care across settings
  • Must have driver’s license and reliable source of transportation and insurance.

  • Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals

Responsibilities

  • Provide comprehensive palliative care consultations across inpatient, community, and home-based settings
  • Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
  • Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
  • Serve as a clinical leader within the interdisciplinary team
  • Educate patients and families on disease progression, treatment options, and goals of care
  • Demonstrate strong time management while managing a diverse patient caseload
  • Participate in interdisciplinary meetings, case conferences, and care planning
