Inpatient Palliative Care Nurse Practitioner / Physician Assistant Palliative Care | California Are you seeking schedule flexibility, meaningful patient relationships, and relief from traditional clinic burnout? Senior Doc is expanding our Inpatient Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for patients with serious or chronic illness. Requirements

• Active California Nurse Practitioner or Physician Assistant license
• Strong interdisciplinary team leadership skills
• Excellent time management and organizational abilities
• Comfortable with home visits and community driving
• Strong communication skills with patients, families, and care teams
• Ability to work independently while coordinating care across settings
• Must have driver’s license and reliable source of transportation and insurance.

Nice To Haves

• Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals

Responsibilities

• Provide comprehensive palliative care consultations across inpatient, community, and home-based settings
• Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
• Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
• Serve as a clinical leader within the interdisciplinary team
• Educate patients and families on disease progression, treatment options, and goals of care
• Demonstrate strong time management while managing a diverse patient caseload
• Participate in interdisciplinary meetings, case conferences, and care planning
