About The Position

NJ Palliative Care Solutions is seeking a Nurse Practitioner or Physician Assistant to provide hospital-based palliative care at RWJ University Hospital for patients living with serious illness. This role offers a structured mentorship program, physician/APP backup, and a collaborative interdisciplinary team. The position focuses on delivering high-touch palliative care, managing complex symptoms, leading goals-of-care conversations, and completing timely documentation. The role involves partnering with the interdisciplinary team and hospital teams, integrating into hospital workflows, and shaping the palliative medicine practice. A significant aspect of the role includes managing transitions from inpatient to the patient’s next setting, considering their longitudinal preferences and best interests.

Requirements

  • Active New Jersey Nurse Practitioner or Physician Assistant license.
  • National board certification.
  • DEA + CDS registration, or ability to obtain by start date.
  • 2+ years of palliative care experience required.
  • Strong communication skills.
  • Strong clinical judgment.
  • Compassion.
  • Comfort working on interdisciplinary teams.

Nice To Haves

  • Palliative care certification (e.g., ACHPN / CHPN) preferred.

Responsibilities

  • Perform best practice, holistic, patient-centered, evidence-based interdisciplinary palliative medicine.
  • Provide palliative care consultations and follow-up visits across the hospital continuum, including ED and ICU encounters, and ongoing management on medical, surgical, and specialty units.
  • Manage complex symptoms for patients with advanced illness and chronic disease.
  • Lead goals-of-care and advance care planning conversations with patients and families.
  • Coordinate with partner SNFs and/or hospices as appropriate.
  • Partner with the interdisciplinary team and hospital teams, fully integrating into hospital workflows and shaping the palliative medicine practice.
  • Complete timely documentation aligned with clinical, billing, and quality standards.
  • Manage transitions from inpatient to the patient’s next setting, considering each patient’s longitudinal preference and best interests.
