Inpatient Clinical Pharmacist
About The Position
The Inpatient Clinical Pharmacist is an integral member of the care team providing medication expertise for drug information, clinical decision support, guideline-based care and supportive transitions of care. This inpatient clinical pharmacist works collaboratively in a decentralized model supporting patients, providers and nurses. On site BLS, ACLS and PALS certifications provided. Requirements
- Completion of an ASHP accredited pharmacy residency preferred
- Licensed as a pharmacist in the State of Washington
- Minimum one (1) year of pharmacy experience preferred in setting appropriate to work assignment (hospital, retail, clinic or other)
Responsibilities
- Dispense medications to patients and interpret IV admixture orders and prepare primary IV/solutions
- Manufacture nonstandard pharmaceutical preparations when required and maintain appropriate records
- Supervise support personnel and assumes responsibility for final check of all completed work
- Assist in the inspection of all pharmaceutical supplies on all services
- Provide for the safe and accurate provision of pharmaceutical care service
