About The Position

The Inpatient Clinical Pharmacist in our organization has a wide array of responsibilities relating to direct patient care. Our clinical pharmacy service is highly valued by our physicians and nursing staff. It is the expectation of the pharmacist to participate in clinical decision-making, assisting in the development of clinical pathways, as well as providing nursing and physician education.

Requirements

• Graduate of an Accredited School of Pharmacy
• Licensed as a Pharmacist in the State of Minnesota
• Intermediate Computer Skills: Word, Excel, Outlook, and Patient Information Database (Cerner and PioneerRx)

Responsibilities

• Supervise and coordinate activities of the pharmacy support staff with respect to accurate and efficient medication dispensing, maintaining records, inventory, and security.
• Medication reconciliation, rounding with physicians, reviewing patient profiles, and making recommendations including drug therapy adjustments, dosing, hyperalimentation monitoring, and addressing specific drug information requests by medical and nursing staff.
• Verifying or entering physician orders, supervising, checking, and certifying activities of pharmacy technicians, maintain prescription and controlled substance records as required, extemporaneous compounding, and prescription processing.
• Special projects may include developing of clinical pathways, participating in clinical meetings, updating policies and procedures, performance improvement, precepting students, and building and maintaining pharmacy technology databases.
• Other duties as assigned.
