Provides pharmacy services and ensures the appropriate selection, dosing, administration, and dispensing of medication orders. You will appropriately select, dose, administer and dispense medications as indicated, helping provide the treatments they need to improve their health and well-being. A caregiver in this position works rotating days/evenings from 7:00AM - 3:30PM or 3:00PM - 11:30PM, with every 4th weekend and two holiday requirements.

Requirements

PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy
Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire

Board Certification
Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology).

Responsibilities

Provide safe, efficient, patient-centered, evidence-based medication services.
Verify medication orders and/or dispenses medications accurately according to state and federal regulations.
Document services in a complete, accurate and timely manner.
Actively contribute to medication safety initiatives.
Ensure all pharmacy operations are in compliance with national accreditation standards.
Directs staff appropriately to provide safe and efficient care and maintain professional working relationships with all caregivers.
Educate and train students, residents, other caregivers and/or patients.
