Inpatient Case Manager/ Registered Nurse (RN) - Part Time Weekend Dayshift
About The Position
Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN) - Part Time Weekend Dayshift. This position is eligible for up to a $10,000 sign-on bonus for qualified candidates. The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. This role directly supports patients in the hospital identified as needing comprehensive case management. The position promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. The Case Manager develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.
Requirements
- Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
- BLS within 90 days of hire.
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
- 1 year of nursing experience
Nice To Haves
- Bachelor's Degree in Nursing preferred
- 1 year acute care case management preferred
