Inpatient Case Manager/ Registered Nurse (RN)
About The Position
Sentara Princess Anne Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN). This is a full-time, permanent position with standard working hours of 8:00 AM – 4:30 PM EST, Monday–Friday, with rotating weekends. The Inpatient Case Manager RN is responsible for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. The primary focus is on patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay (LOS). This role directly supports hospital patients identified as needing comprehensive case management, promotes effective utilization and monitoring of health services, and collaborates with the healthcare team and patient/caregiver to manage care and transitions. A comprehensive care plan is developed based on assessment and evaluation, including physical, mental, and psychosocial needs of the inpatient/caregiver. This position functions within hospitals.
Requirements
- Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
- BLS within 90 days of hire.
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
- 1 year of nursing experience REQUIRED
Nice To Haves
- Bachelor's Degree in Nursing preferred
- 1 year of acute nursing experience or care case management experience preferred
Responsibilities

- Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.
