About The Position

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN) for a Part-Time Weekend Dayshift position. This role is eligible for up to a $10,000 sign-on bonus for qualified candidates. The position is permanent and involves 32 hours per week, with standard working hours of 8:00 AM – 6:30 PM EST on Fridays, Saturdays, and Sundays, or Saturdays, Sundays, and Mondays. This includes one 12-hour shift and two 10-hour shifts, with a 30-minute lunch break. The position is eligible for a weekend differential. Initial training will occur Monday through Friday from 8:00 AM – 4:30 PM EST for several weeks before transitioning to the part-time weekend schedule. This is an onsite position located at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507.

Requirements

• Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
• BLS within 90 days of hire.
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
• 1 year of nursing experience

Nice To Haves

• Bachelor's Degree in Nursing preferred
• 1 year acute care case management preferred

Responsibilities

• Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
• Directly support hospital patients identified in need of comprehensive case management.
• Promote effective utilization and monitoring of health services.
• Collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.
• Functions in one of the following practice settings: Hospitals Only
