About The Position

Sentara Obici Hospital is hiring an Inpatient Case Manager, Registered Nurse. This role is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. The primary focus is on the patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay (LOS). This position directly supports those in-hospital patients identified in need of comprehensive case management. The Inpatient Case Manager promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. They develop and/or implement a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. This role functions in a hospital setting only.

Requirements

  • Registered Nurse (RN) License - Compact/Multi-State License OR Registered Nurse (RN) License - Single State – Virginia
  • Basic Life Support (BLS) - Certification - American Heart Association (AHA)
  • 1 year of nursing experience
  • BLS within 90 days of hire.
  • Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Nice To Haves

  • BSN is preferred.
  • 1-year acute care case management experience preferred.
  • 1 year acute care experience
  • 1 year case management experience
