About The Position

Shady Grove seeks to hire an Inpatient Care Navigator Social Worker for our Care Navigation Team who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.

Requirements

  • Master’s degree in social work, counseling, psychology, or a related field required.
  • Minimum of 2 years of experience in behavioral health care management, case management, or a related field, with a strong understanding of inpatient behavioral health settings.
  • Current licensure in a relevant field (e.g., LMSW, LPC) required

Nice To Haves

  • Certification in care management or behavioral health (e.g., CCM, PMH-C) is a plus.

Responsibilities

  • Conduct comprehensive assessments to identify patients’ needs, strengths, and barriers to care. Develop individualized care plans that promote recovery and address psychosocial factors.
  • Collaborate with psychiatrists, therapists, social workers, and other healthcare professionals to ensure integrated and effective treatment. Facilitate communication among team members to support continuity of care.
  • Identify and connect patients with appropriate community resources, including support groups, social services, and educational programs, to enhance their recovery journey.
  • Provide education and emotional support to patients and their families regarding treatment options, coping strategies, and available resources. Address any concerns or questions they may have.
  • Regularly monitor patient progress and adjust care plans as needed. Schedule follow-up appointments and ensure patients are adhering to treatment recommendations.
  • Maintain accurate and up-to-date records of patient interactions, care plans, and referrals in compliance with facility policies and regulatory requirements.
  • Participate in quality assurance initiatives, gather data on patient outcomes, and contribute to the development of best practices for outpatient behavioral health care.
  • Respond effectively to crisis situations, providing support and stabilization for patients in distress.
