About The Position

The Inpatient Care Manager plays a critical role in coordinating, monitoring, and evaluating patient care throughout the hospitalization and discharge process. This role collaborates closely with physicians, nurses, social workers, and other healthcare professionals to ensure the delivery of high-quality, cost-effective care and the safe transition of patients to the next level of care.

Requirements

Registered Nurse (RN)
Experience in care coordination, utilization review, or case management.
Proficiency in electronic medical records (EMR) systems.
Strong collaboration and communication skills.
Ability to lead and participate in interdisciplinary teams.
Knowledge of discharge planning and continuum of care.

Nice To Haves

Experience in an inpatient setting.
Familiarity with psychosocial outcomes.
Understanding of healthcare resource utilization and length of stay monitoring.

Responsibilities

Manage, coordinate, and facilitate care delivery through comprehensive patient and family assessments.
Collaborate with multidisciplinary teams to support optimal clinical and psychosocial outcomes.
Conduct daily chart and utilization reviews to ensure appropriate level of care.
Maintain accurate, timely, and comprehensive clinical documentation in the electronic medical record (EMR).
Lead and participate in interdisciplinary care rounds.
Identify and proactively address quality-of-care concerns, barriers to progression of care, and discharge delays.
Formulate, coordinate, and implement individualized discharge plans.
Ensure safe, timely, and effective transitions across the continuum of care, including post-acute services and community resources.
Monitor healthcare resource utilization and length of stay.
Partner with the Supervisor/Manager of Case Management to evaluate outcomes and improve care delivery processes.
