About The Position

Atlantic Health System is seeking a dynamic Registered Nurse to join the energetic and innovative team of healthcare professionals as an Inpatient Care Manager on the Care Management Team at Morristown Medical Center. This is a per diem position in which the candidate will provide services during the day, Monday through Friday, with occasional weekends. The ideal candidate will be responsible for providing care and services to selected member populations across the continuum of illness, ensuring effective utilization and monitoring of healthcare resources, managing the interdisciplinary team to achieve optimal clinical and resource outcomes, and partnering with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize the health and well-being of the patient. Responsibilities

  • Manage, coordinate, and facilitate care delivery through comprehensive patient and family assessments.
  • Collaborate with multidisciplinary teams to support optimal clinical and psychosocial outcomes.
  • Conduct daily chart and utilization reviews to ensure appropriate level of care.
  • Maintain accurate, timely, and comprehensive clinical documentation in the electronic medical record (EMR).
  • Lead and participate in interdisciplinary care rounds.
  • Identify and proactively address quality-of-care concerns, barriers to progression of care, and discharge delays.
  • Formulate, coordinate, and implement individualized discharge plans.
  • Ensure safe, timely, and effective transitions across the continuum of care, including post‑acute services and community resources.
  • Monitor healthcare resource utilization and length of stay.
  • Partner with the Supervisor/Manager of Case Management to evaluate outcomes and improve care delivery processes.
