The Inpatient Care Manager Lead is responsible for conducting complete assessments, establishing appropriate plans, and initiating timely interventions. This role involves effective collaboration and negotiation with patients, families, and the care team to achieve patient and organizational goals related to care needs, choices, and satisfaction during discharge planning and care transitions. The Lead acts as a member of the multi-disciplinary care team, promoting best practices in a collaborative and proactive manner. They also conduct concurrent and retrospective clinical reviews, provide clinical updates for certification to third-party payers, and refer cases to a physician advisor as needed. The position requires collecting accurate data on outcomes and resource utilization, facilitating reporting, and serving as an expert user of care management IT platforms. Additionally, the Lead assists the Manager with Human Resources responsibilities, including interviewing, staff development, performance evaluations, and employee relations, while providing general work direction and guidance to case managers. They also serve as an educator and resource for staff on utilization issues and regulatory requirements, collaborate with management on program development, and act as a liaison with physicians on utilization management and care coordination. Monitoring assigned areas for efficient use of supplies and equipment, and determining clinical equipment needs are also key responsibilities. The role requires demonstrating knowledge and skills for age-appropriate patient care, understanding principles of growth and development, and assessing patient status based on age-specific needs.
