The Inpatient Care Manager Lead is responsible for conducting complete assessments, establishing appropriate plans, and initiating timely interventions. This role involves effective collaboration and negotiation with patients, families, and the care team to achieve patient and organizational goals related to care needs, choices, and satisfaction during discharge planning and care transitions. The Lead works as a proactive member of the multi-disciplinary care team, promoting best practices. Responsibilities include concurrent and retrospective clinical review, providing clinical updates for third-party payer certification, referring cases to a physician advisor as needed, and collecting accurate data on outcomes and resource utilization. The Lead also serves as an expert user of care management IT platforms and assists the Manager with Human Resource responsibilities, including interviewing, staff development, performance evaluations, and conflict resolution. They provide general work direction and guidance to case managers, acting as an educator and resource for utilization issues and regulatory requirements. Collaboration with management on new and updated programs/services is expected, along with assisting in the internal management of Care Management, including staff caseload oversight. The Lead also serves as a liaison with physicians on utilization management and care coordination issues and monitors assigned areas for efficient use of supplies and equipment, determining and approving clinical equipment needs.
